All home and telehealth consultations have a $50 concierge fee that is not covered by insurance. This concierge fee covers the cost of travel (under 20 miles) and online platforms as well as the extensive time our lactation consultants spend evaluating both mother and baby at each visit. The fee is included in the self-pay rates shown above, but will be broken down separately at time of booking.

Cash, check, Apple pay or credit/HSA cards are accepted at consultation, or, if desired, pay when you book online. Tips are welcome but not expected. A concierge fee of $50 is applied to all home and telehalth visits and not covered by insurance.*

We are an in-network provider with Aetna. In order for us to submit claims on your behalf, please read and complete this form.
We are an in-network provider for Anthem (telehealth only) and Cigna via Wildflower Health. In order for us to file insurance, please read and complete this this form.
We are an in-network provider with United Healthcare and Multiplan via The Lactation Network (TLN). You can fill out this form here to find out if you qualify for insurance covered visits.
We are an authorized out-of-network provider with Tricare East. In order for us to submit claims on your behalf, please read and complete this form.
We are out of network with Blue Cross Blue Shield, but can provide you with a superbill (medical receipt) you can submit to your insurance for reimbursement.

Please note that $50 concierge fee* applies to all home and telehealth consults and is not covered by insurance.
We are in-network with Aetna. In order to submit a claim on your behalf, you must fill out our release for filing health insurance form within 24 hours of your appointment. Please note, Aetna routinely uses industry standard cost-sharing practices, which may mean that a portion of our rate will out of pocket, especially after 6 visits. However, we do our best to bill lactation consults as a preventative (covered) service.

We are an in-network telehealth-only provider for Anthem via Wildflower Health. To check your plan's eligibility fill out this form.
We are an in-network provider for United Healthcare and Multiplan via The Lactation Network (TLN). Fill out the The Lactation Network form to find out if your plan qualifies. If you have an appointment scheduled, please mark the form as Emergent or Urgent and you should receive a response within a few hours confirming your plan's eligibility. If you do not need an immediate appointment, select Non-Urgent or Planning Ahead and you'll receive a response within 3-4 days.
We are an authorized out-of-network provider with Tricare East. For Tricare members, we require partial payment upfront, and will submit a claim on their behalf afterwards. In order to submit a claim on your behalf, you must fill out our release for filing health insurance form within 24 hours of your appointment.
While your approval is pending, go ahead and book a Lactation Consult on our schedule.
For all other plans, we recommend submitting a superbill (medical receipt) to your insurance company after your consult to request reimbursement. Read our instructions here.
